Transition

What is Transition and what does it mean

Transition is a collaborative effort between the school, the student, the family, and community agencies to help kids with disabilities transition from school to adulthood. It is an ongoing and results-oriented process that includes a commitment of resources, collaboration between people and agencies, and decision-making to develop an IEP for the student.

What is ELEMENTS?

ELEMENTS is a program designed for students (aged 18-22) who have graduated with an applied studies diploma. This transition program gives them the extra instructional time they need to be a more independent adult. The ELEMENTS Program is designed to bridge the gap between high school and adult living by providing students with direct instruction in Independent Living & Job Readiness Skills.

What does the program consist of?

In ELEMENTS, we learn by doing. ELEMENTS students work at various job sites on campus and in our community to learn a variety of job skills. They spend time working on academic and functional skills in an authentic learning environment. Our students are working on adult living skills like cooking and kitchen safety. They learn to stay healthy and active. And they are learning how to be active members of their community through civic duty and volunteering. In ELEMENTS, each student gets the individualized instruction they need to become a more independent adult when they complete the 2+ year program.
